3. STEP 3: Complete the application by filling in the required information
4. STEP 4: Include the completed application with a check payable to the 'Tampa Bay Boxer Club' in a mailing envelope
5. STEP 5: Mail the envelope to the 'Mail To:' address found at the bottom of the application
